Bodyshop options

Why is it that there are more options in bodyshop than the actual game? (Thicker upper lip, thinner/wider eyes, etc) there are many options not in my CAS screen! Can I change this?

It is by design and no, you cannot change it unless you know how to reprogram the entire game!
