This user has the following games installed:

Sims 2, University, Nightlife, Open for Business, Pets, Seasons, Bon Voyage, Free Time, Apartment Life
Default Default replacement just won't go
I've already deleted this underwear in replacement for this underwear but the first one is the one I see in game. Only the second one is in my downloads folder so this just doesn't make sense. Any help is appreciated

You may need to delete your cache files.

The thumbnails don't automatically update. Click on the underwear and look on the mannequin - what do you have? If you see the original outfit, you just need to refresh your thumbnails. If the default replacement is still there, then you haven't removed it - it's still in there somewhere, still overriding the original file. And you'll have to ferret it out - somehow.

I suppose I could have 2 but I've even redownloaded just to check the file name then searched that in my downloads folder so I just figured it wasn't in there, I never rename anything

EDIT: just thought I'd include that I did look at the mannequin on body shop and in game and its still the old replacements
