Putting teen clothes in enabled for maternity will fix this problem.

A cursory search of the downloads right here reveals this:
http://www.modthesims.info/download.php?t=373053

The blog that tracks TS3 maternity-enabled clothes has a section of teen maternity clothes:
http://sims3maternity.blogspot.com/

Poppy sims also has a teen maternity section:

can you have multiple outfits in the mods folder or can you only choose one?

You can have more than one. They're just teen clothes with pregnancy meshes.

Do you put poppy sims meshes in the mod folder, like the maternity clothing?

Yes, if they're .sims3pack just put them in your downloads, if .package then mods folder.

if your sims are not wearing the clothes at the point of conception, does the floaty head thing happen or does it automatically change them into maternity (upon finding out) like adult sims?

what you do is go to electronic arts in the documents section click on that and there will be another folder called sims 3 open that and then in the sims 3 folder create another folder called mods then in the mods folder open it and there will be a folder called packages mods will be .packages and all u have to do is put the packages in the package folder and they should be in your game its a lot of words but i hoped this helped
