If BV jewelry has tongue peircings, you could make an accessory and change it into BV jewelry with the Wardrobe Wrangler. I don't recall if it does or not. We don't have info about the facial skeleton, so we can't actually assign something to the tongue in TS2. So existing tongue studs in BV, if there are such, would be your best bet.

Or you can try doing this as a texture rather than a mesh, and make it a multiwearable blush. That can certainly be done.
