adding parts to a mesh
Try making a copy of a mesh group, chop out a little bit of it, delete the rest.
Shape it how you want it, adjust the normals, adjust the uv mapping, adjust the bone weights.
Then use "regroup" to add it to any mesh group you want.
After regroup, make sure to rename the mesh group and fix the comment!

edit:
You MUST do this for all your morphs (fat, pregnant) too, or your mesh will break! The regular mesh and the morphs must have the same number of vertices and faces. So if you add stuff to your regular mesh, you must add the exact same amount of stuff to your morphs.

bone assignments
I prefer demon's Extended Manual Edit for bone assignments.
Others like the bone tools. Both are in the Vertex menu.
Look in BodyShop -> Meshing -> Tools for these plugins.
You can color your mesh by bone assignment to help you see what's been done so far.
Go to the joints tab, click the checkbox that colors by joint or bone.

To add a collar to a suit, it's probably best to find a suit or a shirt with a collar, cut it out, and then combine that piece into your existing mesh. That way, all the bone assignments should be there and the bone weights will be correct.

To combine groups, you basically just select the two groups you want to combine and hit the "Regroup" button (it's under the Groups tab). Then rename your combined group (to "top" or "bottom" or "body"... depending on what you're making) and adjust the comments. All of this should be covered in the beginner tutorials.

Before you do that, though, make sure you move the UV map for your collar to a new place that's not overlapping some existing part of the UV. Otherwise, you're going to have trouble texturing it.

If you're going to make a new collar from scratch... well, that's going to be a lot of work. You'll need to copy the bone assignments and weights from a similar mesh to make sure everything moves properly. That's about all I can suggest for that. It's going to take you a while to do, that's all!

Also, when adding new parts to a mesh, don't forget to do the same for all morphs BEFORE exporting. Can't stress this enough. If you only add to the main mesh group, not the morphs, then export, your exported mesh will be borked.
